Here is guide to the steps you need to take to place your loved one at Bianchi Manor. We are available to counsel and assist you through this process.
- Take a tour of Bianchi Manor and ask any questions you may have about the facility and our services.
- Make a decision regarding the appropriateness and "fit" for your loved one in consultation with health professionals and family.
- Have the doctor complete a "physician’s report." This will include a TB test. Often you will need to make an appointment to get this done.
- Arrange for an assessment of your loved one by one of our authorized staff.
- Fill out some papers: Emergency form, release of medical information, etc…. Please do not hesitate to ask for our assistance completing these forms. As a licensed care community we are required to have them.
- Make medication arrangements. You will need to decide on a pharmacy of choice and arrange to have medications ready on move-in day.
- Arrange the move-in day. Talk with our management to determine a target day. Some things to consider: Will you bring your own furniture? What support do you need during the move? What is a good time of day for your loved one?
- A photo of the new resident and copies of their social security card, photo I.D., insurance cards, Durable Power of Health, Durable Power of Attorney, DNR or any other legal documents regarding conservatorship or financial responsibility will be needed.
